Social Services Specialist

At SSM Health, we are seeking a compassionate and skilled Social Worker to join our team. As a Social Worker, you will play a vital role in providing emotional support and guidance to patients and their families, helping them navigate the healthcare system and access necessary resources.
Responsibilities:- Conduct comprehensive assessments and develop treatment plans to address mental, emotional, and behavioral health needs.
- Provide individual and group counseling sessions to patients and families.
- Collaborate with multidisciplinary teams to develop and implement patient care plans.
- Participate in discharge planning and ensure a smooth transition for patients.
- Supervise interns and assigned staff to ensure high-quality care.
- Masters degree in Social Work or related field.
- Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) in Illinois, Missouri, Oklahoma, or Wisconsin.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
