Position Overview:
The Clinical Care Coordination Nurse plays a pivotal role in managing patient care through effective collaboration with various healthcare professionals, including medical staff, nursing teams, and support departments.
Key Responsibilities:
- Facilitate comprehensive patient care planning, focusing on discharge strategies and continuity of care.
- Ensure optimal utilization of hospital and community resources while monitoring patient outcomes through ongoing assessments and proactive planning.
Qualifications:
- Must hold a current Registered Nurse license issued by the North Carolina Board of Nursing; a Bachelor of Science in Nursing (BSN) is preferred.
- A minimum of two years of clinical experience in a hospital environment is required.
- Prior experience in utilization review or discharge planning is advantageous.
- Strong organizational and communication skills are essential.
Physical Requirements:
- Ability to perform a full range of motion, including bending, stooping, standing, and sitting for extended periods.
- Must be capable of lifting and carrying weights up to 30 pounds.
