Responsibilities- Provide direct patient care to residents in a Long Term Care facility
- Assess patient needs and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless care delivery
- Document patient progress and maintain accurate records
To be successful in this role, you will need:
- A current RN license in the state of West Virginia
- At least 1 year of experience in a Long Term Care setting
- Excellent communication and interpersonal skills
- Able to work 40 hours per week, with 12-hour shifts
