Part-Time Nurse Practitioner, In-Home Visits

Optum is seeking a skilled Nurse Practitioner to join our Home-based Medical Care team in Boone County, AR.

This position is 10 hours/week, providing an opportunity for a flexible work schedule.

As a member of the broader Home and Community Care team, you'll help bring home-based medical care to complex, chronic patients, giving older adults more days at home.

At Optum, our integrated medical teams create a seamless health journey for patients across care settings, addressing physical, mental, and social needs.

Key Responsibilities:

  • Establish and maintain effective relationships with patients, caregivers, and healthcare providers.
  • Perform comprehensive assessments and provide ongoing care, including visits to address changes in condition.
  • Develop patient management care plans and update them as needed.
  • Consult with Physician Leaders and coordinate with Primary Care Physicians.
  • Perform basic procedures of episodic nursing care, including IV placement, blood draws, and wound care.
  • Participate in HEDIS measure campaigns and quality initiatives to ensure high standards of care.
  • Participate in clinical after-hours on-call program on a rotational basis.

Requirements:

  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Current, unrestricted NP or PA licensure in the State of Practice.
  • Active, unrestricted RN licensure in the State of Practice.
  • Access to reliable transportation.

Preferred Qualifications:

  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, palliative care, emergency/urgent care, or similar settings.
  • Proven knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.
  • Proven knowledge of CPT, ICD-10, and HCPC codes.
  • Proven knowledge of clinical standards of care.
  • Demonstrated awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems.
