LOCATION: Family Medicine Practice
HOURS: 32 hours weekly, including one evening shift and rotating Saturday duties
POSITION SUMMARY: This role involves delivering nursing care to patients within a medical office setting. The scope of care encompasses various age groups: pediatric (infants to 12 years), adolescent (13 to 17 years), adult (18 to 64 years), and geriatric (65 years and older).
KEY RESPONSIBILITIES: Candidates must possess the capability (with or without reasonable accommodation) to fulfill the following tasks:
- Offers clinical guidance via phone or electronic communication based on clinical assessments and triage protocols, adhering to regulatory standards and best practices. Collaborates with healthcare providers as needed.
- Delivers tailored patient education covering preventive care, chronic disease management, acute conditions, medications, and other relevant topics impacting patient outcomes.
- Prepares patients for consultations using established rooming protocols.
- Administers prescribed injections and medications in compliance with departmental protocols and regulatory standards.
- Assists healthcare providers with various procedures, minor surgeries, and examinations as directed.
- Conducts Point of Care Testing, including but not limited to hearing, flu, urine, and strep tests.
- Collects blood or other lab specimens and prepares them for analysis.
- Manages prescription refills as per provider instructions.
- Facilitates tracking of tests and monitors overdue results.
- Performs suture removal and dressing changes as necessary.
- Ensures examination rooms are adequately stocked and sanitized after each patient visit. Maintains and cleans medical instruments and equipment post-procedure.
- Supports providers and patients in reviewing and adjusting care plans while assessing patient progress. Follows up with patients as required between visits.
- Engages in proactive outreach for patients needing preventive and chronic disease services.
- Coordinates care activities among specialists and manages transitions of care to gather all necessary patient information.
- Assists in organizing referrals, education, and community resources.
- Actively participates in initiatives aimed at enhancing patient experience and quality of care.
- Engages in daily briefings with the healthcare provider.
- Acts as a patient experience advocate, maintaining a positive demeanor during interactions with patients, visitors, and colleagues. Communicates professionally while prioritizing customer service.
- Handles all patient information confidentially in accordance with privacy policies and procedures.
- Performs various administrative duties as required.
- May involve some regional travel - a valid driver's license is necessary for travel.
- Participates in on-call duties as needed.
- Other responsibilities as assigned.
MINIMUM REQUIRED QUALIFICATIONS:
- Completion of an LPN or RN educational program.
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) from the state nursing board.
- AHA CPR/BLS certification required within 30 days of employment.
- This position necessitates occasional regional travel. Ability to travel to multiple locations is essential.
- One (1) to Two (2) years of nursing experience in a primary or specialty care setting.
- Prior experience in triage, emergency care, cardiac care, intensive care, or other relevant nursing specialties.
