Speech-Language Pathologist Home Health Specialist

EvergreenHealth is seeking a skilled Speech-Language Pathologist to provide high-quality care to patients in their homes. As a member of our interdisciplinary team, you will be responsible for assessing, planning, implementing, and evaluating the care of patients requiring speech therapy services.
Primary Duties:
* Provide skilled intermittent care to patients and educate them and/or their caregivers to promote maximal functional status.
* Regularly evaluate and re-evaluate patient status to ensure the best possible outcomes.
* Develop and implement individualized plans of care (POCs) that address the unique needs of each patient.
* Communicate effectively with other healthcare professionals involved in the patient's care to ensure seamless coordination and delivery of services.
Requirements:
* Current licensure to practice as a Speech-Language Pathologist in Washington State.
* Recent clinical experience in a medical/surgical unit, rehab, or home health setting.
* Current Washington State driver's license and proof of insurability.
* Current Healthcare Provider BLS certification by date of hire.
* Reliable transportation.
