As a leading provider of services for multiply disabled adults who have faced chronic homelessness, DESC supports nearly 2,000 individuals daily. DESC is recognized both nationally and regionally for its innovative approaches to addressing homelessness.

POSITION SUMMARY:

The Clinical Nurse Specialist is an integral member of the SAGE (Support, Advocacy, Growth and Employment) multidisciplinary team, which includes nurses, psychiatric providers, case managers, substance use counselors, employment specialists, and peer specialists. This role is focused on delivering comprehensive case management services to homeless and formerly homeless adults with mental health challenges. The Clinical Nurse Specialist primarily engages with clients requiring psychiatric and medical care, overseeing medication coordination/management, client education, health assessments, and related services, all under the supervision of the Clinic Nursing Supervisor.

KEY RESPONSIBILITIES:

Case Management & Social Support:

  • Collaborate as part of a multidisciplinary team to provide ongoing case management services to homeless and formerly homeless adults.
  • Evaluate clients for a variety of healthcare needs and facilitate connections to urgent, primary, and specialty health services.
  • Advocate for clients' access to community resources and services, particularly primary healthcare, ensuring their needs are met and rights upheld; consult and collaborate with community providers to maintain continuity of care.
  • Contribute to the planning, organization, and facilitation of group psycho-educational treatment services for individuals served by the mental health program.
  • Coordinate services closely with mental health program supervisors, case management staff, chemical dependency treatment staff, and other DESC personnel.
  • Engage in outreach to clients at various community locations providing homelessness services.

Medication Management:

  • Under the guidance of the Clinic Nursing Supervisor, oversee the medical aspects of medication services for clients.
  • Deliver individualized medication management services and education as necessary, along with ongoing assessments of mental status and treatment impacts.
  • Provide specialized training and consultation to other staff members regarding medical issues related to clients and their care.
  • Participate in psychiatric consultations, supervision, program meetings, and in-service training; engage in clinical reviews and case conferences as required.
  • Adhere to relevant professional regulations, the agency's clinical accountability policies and procedures; maintain current and comprehensive clinical records; participate in quality assurance reviews.
  • Comply with applicable program research and evaluation protocols.
  • Perform other duties as assigned.

Qualifications

MINIMUM REQUIREMENTS:

  • Registered Nurse licensed in the State of Washington.
  • Experience working with homeless, mentally ill, and/or substance-abusing populations is preferred.
  • Knowledge of psychiatric medications, their side effects, and administration methods.
  • Experience engaging clients who may be resistant to traditional mental health programs.
  • Ability to effectively interact with clients exhibiting a wide range of challenging behaviors.
  • Strong communication skills and the ability to work effectively with a diverse staff.
  • Commitment to the philosophy of cooperation and continuity across programs, with respect and consideration for clients.

PREFERRED QUALIFICATIONS:

  • Bachelor's Degree in Nursing.

$103, $114,309.12 annually
