About the Role

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at Chapters Health System. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional patient care and support to individuals and their families.

Key Responsibilities
  • Conduct thorough nursing assessments and develop individualized care plans to meet the unique needs of each patient.
  • Collaborate with healthcare professionals, patients, and families to ensure seamless care coordination and communication.
  • Supervise and mentor LPNs, CNAs, and Patient Care Volunteers to ensure high-quality patient care.
  • Manage patient caseloads, prioritize tasks, and maintain accurate records and documentation.
  • Participate in interdisciplinary team meetings and contribute to the development of care plans and strategies.
Requirements
  • Current licensure as a Registered Nurse in the District of Columbia, State of Maryland, or Commonwealth of Virginia.
  • At least one year of experience as a staff nurse in a comparable setting, with a strong preference for hospice and/or palliative care experience.
  • Bachelor's degree in Nursing from an accredited nursing program, with a BSN degree preferred.
  • Healthcare Provider BLS (CPR) certification and a CHPN certification a plus.
Preferred Qualifications
  • Experience working in a fast-paced environment with multiple priorities and deadlines.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
