Hospice Case Manager Job Description

This role will be located in our Magnolia Hospice Arlington location. We believe in placing the patient's interests at the heart of everything we do, making our job so rewarding. As a Hospice Case Manager, you will spend time genuinely understanding the patient's story, which helps determine what they need to feel comfortable, safe, and healthy in their own home.

Hospice Case Manager General Purpose: To assure quality patient care and services are provided to in-home clients. You will be accountable for making decisions based on your educational preparation and experience in nursing.

Responsibilities:

  • Assessing and providing hands-on care
  • Admissions after hours
  • After hours discharges, including death pronouncement
  • Communicating with the interdisciplinary team
  • Documenting necessary information in the electronic healthcare record
  • Collaborating with the hospice physician
  • Coordinating necessary ancillary services, such as DME, lab, x-ray, transportation
  • Providing services collaboratively with other healthcare providers, such as in nursing facilities, hospitals, and home health
  • Oversight and direction of unlicensed assistive personnel
  • Education and support for family, caregivers, and facility staff

Hospice Case Manager Minimum Education & Experience Requirements:

  • Current RN License – active and in good standing
  • Minimum of 1-year nursing experience preferred
  • Hospice experience is a plus
  • Current CPR certification
  • Strong organizational and communication skills
  • Proof of current auto insurance
