Nursing Home Health Specialist

We are seeking a skilled Nursing Home Health Specialist to join our team at connectRN. As a vital member of our community, you will provide exceptional care to patients in their homes, utilizing comprehensive assessments and creating individualized care plans.
CompensationThis is a rewarding opportunity with a competitive pay rate, including:
- $54.27 per point (weekdays)
- $57.20 per point (weekends)
You will also have the chance to earn exclusive bonuses, mileage reimbursement ($0.585 per mile), and unlimited referral bonuses.
About the RoleAs a Nursing Home Health Specialist, you will be responsible for providing independent care to patients, keeping up-to-date documentation using HomeCare HomeBase, and collaborating with our supportive network of nurses.
RequirementsTo qualify, you must have:
- A current and unencumbered LPN licensure
- At least one year of certified home health experience as an LPN
- Reliable transportation, a current driver's license, and car insurance coverage
- A current CPR certification, with additional certifications in home health or geriatric care preferred
