Psychiatric Care Manager

As a key member of the Behavioral Health team, you will provide comprehensive psychiatric evaluations, medical assessments, and interventions for clients within our programs. Your role will include ongoing medication management and assessments, as well as offering consultation and education to various agencies and disciplines, including other services within VNS Health. You will play a vital role in the overall management of the Behavioral Health program.
Key Responsibilities:
- Conduct thorough psychiatric evaluations for designated clients.
- Analyze current administrative workflows and clinical protocols to identify areas for enhancement in quality and cost-efficiency while ensuring compliance with nursing standards.
- Deliver medication management and supportive counseling on a regular basis.
- Assess the necessity for physical evaluations and medical services, making referrals as appropriate.
- Provide a full spectrum of advanced nursing practice tailored to specific client populations, evaluating responses to therapies and adjusting care plans as needed.
- Collaborate with other mental health professionals and community organizations regarding individual client care.
- Engage in interdisciplinary team meetings and case discussions for the Behavioral Health program.
- Maintain client documentation in accordance with VNS Health and regulatory requirements.
- Evaluate clients for potential psychiatric hospitalization when necessary.
- Offer clinical supervision and guidance to Behavioral Health program staff, including direct observation of patient care.
- Work with program management to ensure the delivery of high-quality behavioral health services, contributing to the development and revision of clinical and administrative policies.
- Encourage client and family involvement in the treatment planning process.
- Participate in community programs and educational initiatives as requested.
- Assist in the identification and resolution of client crises, collaborating with program management.
- Engage in special projects and perform additional duties as assigned.
Qualifications:
Licenses and Certifications:
- Current Nurse Practitioner license and registration in New York State required.
- Board Certification as a Psychiatric-Mental Health Nurse Practitioner required within one year of assuming the role.
- DEA license required.
- A valid driver's license may be necessary based on operational needs.
Education:
- A Master's Degree in Nursing or an equivalent graduate degree from a recognized nurse practitioner program is required.
- A Master's Degree in Psychiatric Mental Health Nursing or a specialized PMHNP degree is preferred.
Work Experience:
- A minimum of three years of clinical experience in a behavioral health setting is required.
- For positions within Certified Community Behavioral Health Clinics (CCBHC), at least one year of experience in substance use treatment, including medication-assisted treatment, is required.
- Excellent verbal and written communication skills are essential.
- Strong proficiency in electronic systems for clinical documentation is required.
- Bilingual proficiency in English and Spanish is preferred.
