We're seeking a highly skilled Dialysis Care Coordination Manager to join our team at Henry Ford Health System. As a key member of our healthcare team, you'll be responsible for coordinating high-quality patient care and ensuring seamless transitions between care settings.

Requirements

To succeed in this role, you'll need:

  • A graduate degree from an accredited nursing program
  • Current RN licensure in Michigan
  • A minimum of 12 months of experience as a registered nurse
  • At least three months of experience in hemodialysis (HD) and/or peritoneal dialysis (PD)
  • Basic computer knowledge and keyboarding skills
