Optimal Care Inc. is a Medicare Certified/CHAP Accredited home health care agency, established in 2006 and owned by nurses. We serve homebound adult patients in Wayne, Oakland, Macomb, and southeast Livingston Counties.
We offer high compensation for excellence, a comprehensive benefit package for full-time employees, including 75% employer-paid benefits, 401K company contribution, paid time off (PTO), and more.

Salary Estimate:$50,000 - $60,000 per year based on experience.
Care Coordinator Responsibilities:
• Coordinate home care services for patients in a timely manner.
• Provide excellent customer service.
• Assist with patient care management through accurate intake data entry and system reporting.
• Coordinate patient and clinical staff scheduling.
• Maintain a good working rapport with community and company partners, interdepartmental personnel, and other departments within the facility.
• Verify accuracy of patient scheduling and data entered through use of information systems reports.
• Enter and update patient data into information systems as required.
• Perform daily backup of information in the system(s) where required.
• Alert management team members regarding late or missing documents required for intake of a patient and data entry.
• Maintain confidentiality of patient and organization information at all times.

Qualifications:
• At least one (1) year experience in healthcare data entry, preferably in home care or hospice care information systems.
• Experience with computers required.
• High school graduate or equivalent, two (2) years college preferred.
