Job Summary

We are seeking an experienced Registered Nurse Case Manager to join our team at Medical City Dallas. As a key member of our multidisciplinary team, you will play a vital role in coordinating patient care and ensuring seamless transitions between hospital stays and community-based services.

Responsibilities:

  • Conduct comprehensive assessments of patient needs and develop individualized care plans to promote optimal outcomes.
  • Collaborate with healthcare providers and social workers to ensure timely referrals to necessary services and resources.
  • Coordinate discharge planning and arrange for necessary follow-up appointments and community-based services.
  • Provide education and support to patients and families on disease management, medication adherence, and lifestyle modifications.
  • Maintain accurate and up-to-date records of patient interactions, treatment plans, and outcomes.

Qualifications:

  • Current Texas state license as a Registered Nurse.
  • At least 3 years of clinical hospital nursing experience.
  • 2 years of case management experience, preferably in a critical care setting.
  • Certification in case management preferred.
