We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.
Key Responsibilities- Evaluate and assess patients' speech, language, and swallowing disorders
- Develop and implement individualized treatment plans to address patients' needs
- Provide instruction and training to patients and their families on the use of alternative communication systems
- Collaborate with healthcare teams to ensure comprehensive care and coordination
- Document patient progress and maintain accurate records
- Master's degree in Speech-Language Pathology or related field
- Current certification in Speech-Language Pathology (CCC-SLP) or Audiology (CCC-A)
- Minimum of six months experience as a speech therapist or speech-language pathologist
- Home Health experience preferred
- Current and unrestricted license to practice in the state
- Current CPR certification
- Good organizational and communication skills
