We are seeking a skilled and compassionate Home Health RN to join our team at HCA Virginia Healthcare at Home. As a PRN Home Health RN, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.
Key Responsibilities- Assess patients' physical, psychosocial, and environmental needs, and develop individualized care plans to address these needs.
- Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
- Participate in on-call rotation and emergency call as needed.
- Basic Cardiac Life Support certification required within 30 days of employment start date.
- Registered Nurse licensure and Associate Degree or Registered Nurse Diploma required.
- 1 year of experience in home health or a related field preferred.
