Job SummaryThis is a shift position that requires the ability to provide medical care in the privacy of patients' homes, with a focus on delivering high-quality patient care and collaborating with healthcare teams to achieve optimal outcomes.
Responsibilities- Conduct thorough nursing assessments of patients and document findings in patient records.
- Communicate effectively with physicians and other healthcare professionals to ensure seamless care coordination.
- Provide and coordinate patient care with other healthcare team members to achieve optimal patient outcomes.
- Monitor patients' responses to interventions and report outcomes to healthcare teams.
- Administer medications as ordered and document appropriately.
- Provide medical care in the privacy of patients' homes, requiring frequent driving to multiple patient locations.
- Share on-call responsibilities with a 30-minute response time if required.
- Perform other duties as assigned by healthcare teams.
- One year of experience as a Registered Nurse, with at least six months of recent Home Health experience.
- Current RN License.
- Current BLS Certification.
- Valid, unrestricted driver's license.
- Valid auto insurance.
- Other certifications or licenses may be required for this position.
- Critical thinking, service excellence, and good interpersonal communication skills.
- Ability to read and comprehend written instructions, follow verbal instructions, and use PC skills.
- Physical Abilities: Remain in a stationary position, move about, move equipment (up to 25 lbs), pushing, pulling, bending.
