Key Responsibilities
  • Provide clinical pharmacy services to patients, including medication therapy management and patient education
  • Collaborate with healthcare teams to develop and implement patient care plans
  • Conduct medication use evaluations and make recommendations for improvement
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures
  • Stay up-to-date with the latest clinical guidelines and research in pharmacy practice
Requirements
  • Doctor of Pharmacy (Pharm.D.) degree from an accredited college
  • Graduate from an accredited college by the Accreditation Council on Pharmaceutical Education (ACPE)
  • Internship/Residency or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy
  • A current, valid, unrestricted license to practice Pharmacy in any state
  • 3 years experience with a National Certificate or Residency Required
  • BLS certification
