Job Summary

We are seeking a highly skilled Behavioral Health Specialist to join our team at Berkshire Farm Center & Services for Youth. As a key member of our staff, you will play a critical role in coordinating rehabilitative and support services for youth and families.

Key Responsibilities
  • Coordinate rehabilitative and support services for youth and families to ensure safe, productive, and independent living within their community.
  • Maintain positive partnerships with referring agencies and staff, ensuring timely and effective communication regarding case activities.
  • Provide rehabilitative and supportive services for youth and families under Medicaid Managed Care CFTSS programs in individual and group settings.
  • Offer supportive interventions to youth and families based on treatment, service, and discharge plans.
  • Complete all state-mandated and agency documentation according to policy and regulatory requirements.
  • Assist in ensuring a safe and therapeutic environment by attending internal and external meetings and case reviews.
  • Deliver rehabilitative psychoeducation and supports, strengths-based service planning, and strategies for success.
  • Facilitate personal and community competence, daily living skills, and community integration to restore, rehabilitate, and support youth and families.
Requirements
  • Bachelor's degree in social work or related field required.
  • Minimum of 2 years applicable experience with children (mental health, foster care, child welfare, juvenile justice, or related human services).
  • Demonstrated ability to handle confidential and sensitive information with extreme discretion, knowledge of HIPAA regulations.
  • Valid Driver's License with acceptable driving history to the Agency.
  • Ability to work a flexible schedule, including after-school hours from 3:30p - 7p, evenings as necessary.
