Mass General Brigham is seeking a highly skilled Nurse Navigator to join our team in Malden, Massachusetts.

Job Summary:

We are looking for a dedicated and organized RN to work as a Nurse Navigator for our Allogeneic Hematopoietic Cell Transplant program. The successful candidate will be responsible for guiding patients and their families through the complexities of the transplant journey, ensuring seamless coordination of care and optimal patient outcomes.

Key Responsibilities:

  • Coordinate patient care and services, including pre-transplant work-up, medical clearance, and admission.
  • Collaborate with the Transplant Search Coordinator to initiate Human Leukocyte Antigen (HLA) typing for recipients and potential donors.
  • Work with the Transplant Search Coordinator and NMDP Case Manager to facilitate National Marrow Donor Program (NMDP) unrelated donor workups, including donor or cord blood searches, requesting and prioritizing potential matches, and finalization of product delivery.
  • Act as a primary point of contact for MGH clinicians, advanced practice providers, lab technologists, social work staff, and quality management.
  • Participate in interdisciplinary case review meetings and maintain precise and organized records.

Requirements:

  • Current Registered Nurse licensure in the State of Massachusetts.
  • At least two years of Hematopoietic Cell Therapy experience.
  • Organized and able to communicate clearly with patients, families, and the multi-disciplinary team.
  • Ability to work collaboratively with a job share partner to maintain clear tracking and records of the patient journey.
  • Ability to learn multiple electronic platforms.
