RN Case Manager

We are seeking a highly skilled Registered Nurse to join our team as a Case Manager. The successful candidate will be responsible for providing high-quality nursing care to individuals with developmental disabilities, working collaboratively with our multidisciplinary team to ensure the best possible outcomes.

• Develop and implement individualized nursing care plans in collaboration with the Program Coordinator and other team members.
• Provide 24-hour accessibility and respond to nursing care needs in a timely and effective manner.
• Delegate instructions to unlicensed personnel and Certified Medication Technicians as necessary.
• Conduct regular nursing assessments and establish goals and objectives to address health care needs.
• Provide hospitalization and post-hospitalization intervention, as well as quarterly medical reports and follow-up care plan revisions.
• Assist with development and implementation of orientation and on-going staff training programs, particularly those related to nursing and medical issues.
• Identify and provide ongoing staff training as required by state regulations.
• Comply with Maryland DDA requirements, including completion and review of Health Risk Screening Tool (HRST) online assessments.
• Administer medical and nursing interventions outside the scope of unlicensed staff or deemed inappropriate for delegation.
• Assist with scheduling and coordination of medical appointments and follow-up care.
• Perform site visits to audit nursing assignments, review medication administration, and monitor CMT activities.
• Update health information and implement the agency monitoring system to oversee medication administration and assess competency of skills learned.
• Relay pertinent information regarding medications, dietary recommendations, or medical/nursing issues to the ESRO nurse as indicated.
• Prepare medical reports and attend interdisciplinary team meetings and admission/discharge meetings as necessary.
• Participate in licensing surveys and other service audits or reviews.
• Establish relationships with medical/nursing vendors who provide health care services to individuals supported by Benedictine.
• Maintain confidentiality (HIPAA) of all residents and promote individual rights at all times.

• Associate's degree (or higher) from an accredited nursing program.
• Current Registered Nurse license through the State of Maryland or an accepted multi-state license to practice.
• DDA Delegating Nurse training at the earliest opportunity.
• Minimum of two (2) years of nursing experience, with previous experience working with individuals with developmental disabilities preferred.
• Current CPS training.
• Pass initial and ongoing trainings.
• Minimum of 18 years old.
• Proof of eligibility to work in the US upon offer.
• Pass pre-employment drug screen.
• Pass pre-employment criminal background checks through CJIS as stated in Family Law Article 5-561, and pass pre-employment CPS check.
• Current and valid driver's license in good standing, with a driving record that would permit coverage under Benedictine driver's insurance.
