We are seeking a highly skilled and compassionate RN Case Manager to join our team at Benedictine School. As a RN Case Manager, you will play a vital role in providing high-quality nursing care to individuals with developmental disabilities.
Job Summary:The RN Case Manager will be responsible for planning, developing, and implementing nursing programs to meet the healthcare needs of our clients. This includes providing 24-hour accessibility, delegating tasks to unlicensed personnel, and ensuring compliance with Maryland Nurse Practice Act and DDA regulations.
Key Responsibilities:- Provide 24-hour accessibility and respond to telecommunication requests for quality nursing care.
- Complete nursing assessments, establish goals, and objectives to address healthcare needs.
- Develop and implement nursing care plans, including hospitalization and post-hospitalization interventions.
- Assist with staff training programs, especially those related to nursing and medical issues.
- Comply with Maryland DDA requirements, including Health Risk Screening Tool (HRST) assessments.
- Administer medical and nursing interventions outside the scope of unlicensed staff.
- Perform site visits to audit nursing assignments, review medication administration, and monitor CMT activities.
- Update health information and implement the agency's monitoring system for medication administration.
- Relay pertinent information regarding medications, dietary recommendations, or medical issues to the ESRO nurse.
- Prepare medical reports and attend interdisciplinary team meetings.
- Associate's degree (or higher) from an accredited nursing program.
- Current Registered Nurse license through the State of Maryland or an accepted multi-state license to practice.
- DDA Delegating Nurse training.
- Minimum of two (2) years of nursing experience, preferably working with individuals with developmental disabilities.
- Current CPS training.
- Pass initial and ongoing trainings.
- Valid driver's license in good standing.
