Benedictine School is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for providing high-quality nursing care to individuals with developmental disabilities.
Key Responsibilities:- Develop and implement individualized care plans to meet the unique needs of each client.
- Conduct regular assessments and evaluations to ensure the best possible outcomes for clients.
- Collaborate with interdisciplinary teams to ensure comprehensive care and support.
- Provide education and training to clients and their families on health and wellness topics.
- Document all client interactions and progress in a timely and accurate manner.
- Current RN license in the State of Maryland.
- Minimum of 2 years of experience in case management or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic computer skills and proficiency in electronic medical records.
