About the Role:

Job Summary

This position is designed for individuals who have successfully completed Certified Nursing Assistant training and provide direct personal care and supportive services to patients in their residence under the supervision of a Registered Nurse, following the personalized care plan. The role encompasses caring for patients across all age groups, including infants, children, adolescents, adults, and the elderly. Employees will perform clinical tasks in line with specific population guidelines and adhere to National Patient Safety Goals as per established policies and procedures.

Essential Tasks and Responsibilities

  • Delivers personal care services such as bathing, grooming, nail care, and oral hygiene as outlined in the patient's care plan, ensuring the patient's environment remains tidy and organized.
  • Utilizes appropriate body mechanics to assist with positioning, mobility, and comfort needs, contributing to patient safety and reporting any concerns to the Patient Care Manager/RN/Therapist.
  • Performs light housekeeping tasks in patient residences and engages in other patient-related activities as directed by the RN according to the hospice care plan.
  • Completes an average of 5 billable visits per day.
  • Provides respite and/or continuous care for patients and caregivers as necessary and directed by the RN/Patient Care Manager.
  • Documents care provided in accordance with service protocols and submits to designated personnel within 24 hours of the visit, maintaining 95% accuracy as per record review.
  • Participates in 12 hours of HHA in-service/education programs annually, along with additional in-services as required.
  • Operates a vehicle within the designated service area, adhering to safe driving standards as evidenced by DMV reports.
  • Engages in Performance Improvement initiatives as directed by the Patient Care Manager.
  • Reports any issues or concerns regarding patients to the RN/Patient Care Manager.
  • Assists patients in achieving their Activities of Daily Living (ADLs) goals.
  • Contributes to the development of the Plan of Care by reporting changes in patients' and caregivers' functional levels and capabilities.
