About the Role

Case Management Alliance for Social Empowerment (CASES) seeks a highly skilled and empathetic Mental Health Support Specialist to join our Bronx Nathaniel Assertive Community Treatment (ACT) Team. The successful candidate will provide essential support, services, and treatment to clients with serious mental illnesses, promoting their recovery and community integration.

Key Responsibilities
  • Attend daily team meetings to discuss clients' clinical and case management issues and needs;
  • Document all required information accurately and efficiently, adhering to NYS OMH and NYC DOHMH guidelines as well as CASES ACT Policy and Procedures;
  • Deliver peer services using motivational interviewing, recovery, and trauma-informed approaches to address housing, income support, education, vocational training, social supports, employment, and primary care needs;
  • Collaborate effectively with clients' identified support networks and other service providers for service coordination;
  • Provide individual peer support and wellness counseling, harm reduction counseling, and advocacy services using a person-centered approach;
  • Act as a liaison for clients' access to resources and benefits to support service plan goals;
  • Participate in 24-hour crisis intervention on-call services on a rotating basis;
  • Ensure culturally competent services are provided in accordance with CASES policies and practice.
Requirements
  • A high school diploma or equivalent;
  • Certified Peer Specialist or Certified Recovery Peer Advocate, or eligibility to obtain certification within 6 months of hire;
  • At least 2 years of work experience providing peer support to individuals with serious mental illness and criminal justice involvement;
  • Lived experience using mental health treatment and/or substance use treatment services;
  • Proficiency with computer and electronic health records.
