We are seeking a skilled and compassionate LPN to join our team at HCA Virginia Healthcare at Home. As a Home Health LPN, you will have the opportunity to provide high-quality patient care in the comfort of their own homes.

About the Role

As a Home Health LPN, you will be responsible for administering quality, skilled nursing care to patients in their homes. You will work in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services.

Key Responsibilities
  • Administer skilled nursing care to patients in their homes
  • Work in accordance with physician orders and the Plan of Care
  • Provide patient care and identify the physical, psychosocial, and environmental needs of patients
  • Verify Plan of Care prior to each visit and provide care according to the physician's orders
  • Document all findings, communications, and appropriate interventions
  • Provide basic and specialized teaching as ordered in the Plan of Care and based on the patient's needs
