Job Title: LPN Home Care

**Job Summary:**

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care to pediatric patients in Lexington, KY. As an LPN Home Care Specialist, you will be responsible for delivering personalized care and support to patients in the comfort of their own homes.

**Responsibilities:**

* Provide quality patient care and perform technical skilled care in the patient's home
* Develop and implement individualized care plans in collaboration with the healthcare team
* Conduct thorough assessments and evaluations to identify patient needs and goals
* Educate patients and their families on proper care and management techniques
* Collaborate with other healthcare professionals to ensure seamless care coordination

**Requirements:**

* Current LPN licensure in the state of Kentucky
* At least one year of experience working as an LPN
* CPR certification
* Reliable transportation and a valid driver's license
