About the Role:

As a Licensed Practical Nurse at Holladay Park Plaza, a 5-Star Continuing Care Retirement Community, you will have the opportunity to provide patient-centered care in our 51-bed Skilled Nursing Facility. Our team values compassion, empathy, and excellent clinical skills, and we are seeking a dedicated LPN to join our team.

Responsibilities:

- Provide high-quality nursing care to residents in our Skilled Nursing Facility
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Monitor and report changes in resident condition to the Charge Nurse or Director of Nursing
