LPN Field Position in Home Health Portland Part Time

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Portland, Oregon. As a LPN, you will provide high-quality patient care in a home health setting, working under the supervision of a Registered Nurse (RN) or Medical Doctor (MD).
Responsibilities- Perform procedures that require specialized training for general nursing care and related services for assigned patients.
- Participate in the planning for and provision of patient care, using knowledge of asepsis in performance of treatments.
- Provide functional direction to ancillary personnel.
- Current Licensed Practical Nurse (LPN) license.
- Basic Life Support (BLS) Health Care Provider certification.
- Arrhythmia certification (preferred).
- Valid Driver's License (DL) and must be at least 21 years of age or older.
- Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) certification.
- Provides care for assigned patients, under the supervision of an RN according to established standard and nursing process.
- Performs admission assessments, and ongoing re-assessments based on departmental specific policy and procedures.
- Implements and coordinates plan of care utilizing appropriate organizational skills and reporting mechanisms.
