Position Overview

Objective of the Role

The primary objective of this role is to deliver rehabilitation services to residents, assisting them in enhancing mobility across various levels of care, thereby improving their functional capabilities and overall quality of life. The Physical Therapist will collaborate closely with the interdisciplinary team to assess and treat residents effectively.

Authority Delegation

As a Physical Therapist, you are entrusted with the administrative authority, responsibility, and accountability essential for executing your designated duties.

Key Responsibilities

Core Duties & Responsibilities

  • Assess residents to identify their physical therapy requirements, including their strengths, limitations, and potential for improvement.
  • Design and implement personalized treatment plans aimed at enhancing residents' strength, flexibility, and range of motion.
  • Adjust treatment methodologies as necessary to cater to the unique needs of each resident.
  • Document residents' progress and share findings with the interdisciplinary team and families.
  • Work collaboratively with the interdisciplinary team, including physicians, nurses, and rehabilitation staff, to provide holistic care.
  • Educate residents and their families on strategies and techniques to foster functional independence and prevent further injuries.
  • Maintain accurate and current records for each resident, including progress notes and treatment plans.
  • Ensure a safe and hygienic treatment environment.

Personnel Responsibilities

Adhere to work assignments and schedules while completing your designated tasks. Perform all tasks in accordance with established policies and procedures, as directed by your supervisor. Report any resident complaints or grievances promptly.

Professional Development

Engage in scheduled training and educational programs, including orientation and continuing education, to maintain licensure as a Physical Therapist.

Safety Protocols

Comply with established safety regulations and participate in safety drills. Report hazardous conditions and follow safety precautions diligently.

Equipment Usage

Utilize only the equipment you are trained to operate, ensuring safe usage and reporting any defective equipment to the appropriate personnel.

Resident Rights

Maintain confidentiality of resident information, treating all residents with kindness, dignity, and respect. Ensure residents' rights are upheld at all times.

Qualifications

Educational Background

Bachelor's or Master's degree in Physical Therapy with current state licensure as a Physical Therapist. Experience in skilled nursing facilities or with geriatric populations is preferred.

Work Environment

Work in well-lit and ventilated areas, engaging with residents and staff throughout the facility. Be prepared for frequent interruptions and various interactions with residents, families, and personnel.

Physical Demands

Must be able to lift and push/pull a minimum of 50 pounds, stand and walk for extended periods, and manage the mental and emotional stresses associated with the role.
