We are seeking a skilled and compassionate Registered Nurse to join our Home Care team at Novant Health's New Hanover Regional Medical Center location in Wilmington, NC.
Key Responsibilities:- Coordinate home care services, including establishing and maintaining patient plans of care.
- Provide routine nursing visits as needed and coordinate patient care between patients, caregivers, physicians, and other disciplines.
- Responsible for the completion of all home health documentation.
- Graduate of an accredited school of nursing.
- Associate's or Bachelor's degree in nursing.
- Current valid North Carolina nursing license.
- Minimum 1 year of home health or related experience.
