Home Health Registered Nurse - PRN

We are seeking a skilled and compassionate Home Health Registered Nurse to join our team in Wilmington, NC. As a Home Health Registered Nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Key Responsibilities:

  • Conduct routine nursing visits to assess patient needs and develop individualized care plans.
  • Coordinate patient care with healthcare teams, including physicians, therapists, and social workers.
  • Manage patient documentation and ensure accurate and timely reporting.
  • Collaborate with caregivers and family members to ensure seamless care transitions.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current valid North Carolina nursing license.
  • Minimum 1 year of home health or related experience.
  • Associate's or Bachelor's degree in nursing.
