Overview

Novant Health is seeking a highly skilled Registered Nurse to join our Home Care - Skilled Nursing Unit team in Wilmington, NC. As a Registered Nurse, you will play a vital role in coordinating home care services, ensuring the highest quality patient care and customer satisfaction.

Responsibilities
  • Conduct admissions, recerts, ROC, and discharges for the assigned case load.
  • Conduct routine nursing visits as needed.
  • Assess patient needs and coordinate patient care between the patient, caregiver, physician, and other appropriate disciplines.
  • Responsible for the completion of all home health documentation.
Requirements
  • Graduate of an accredited school of nursing required.
  • Associate's or Bachelor's degree in nursing.
  • Current valid North Carolina nursing license required.
  • Basic Life Support (BLS) through American Red Cross or American Heart Association required.
  • Driver's License Class C.
  • Minimum 1 year of home health or related experience required.
