Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at CenterWell Senior Primary Care. As a key member of our integrated care team, you will provide proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
Responsibilities- Provide high-quality, patient-centered care to seniors in their homes, focusing on personalized experiences and addressing the factors that impact patient well-being.
- Work collaboratively with our integrated care teams, including physicians, nurses, and behavioral health specialists, to achieve optimal patient outcomes.
- Develop and implement comprehensive care plans, taking into account the physical, emotional, and social needs of our patients.
- Prescribe medication, examine patients, diagnose illnesses, and provide treatment, exercising independent judgment and decision-making on complex issues.
- Use laptops for EMR charting and operations support for prescription refills, lab follow-up, and coordinated care management.
- Valid state driver's license, clean driving record, and proof of auto insurance.
- Bachelor's degree and minimum 2 years of clinical experience.
- Active, unrestricted state license, DEA license, and board certification.
- Senior patient care experience and strong technical experience.
- 2 or more years of project leadership experience.
- Active BLS through the AHA or ARC.
- Master's in science of nursing.
- House call experience.
- Urgent care, primary care, family practice, or internal medicine experience.
- Value-based care knowledge and/or experience.
- HEDIS quality metrics knowledge and/or experience.
