Job Description

As a Community Outreach Coordinator, you will play a vital role in coordinating supportive services for residents in one of the many communities that PSS serves. Your primary responsibility will be to connect with community members, build partnerships, and conduct outreach to ensure that individuals are aware of the services offered through the NY Connects program.

You will engage with local organizations, community groups, and individuals to expand awareness of available resources for older adults and individuals with disabilities. This role is crucial in helping bridge the gap between community members and the services they need to live independently and with dignity.

Responsibilities

  • Develop and maintain relationships with community-based organizations, healthcare providers, senior centers, and local advocacy groups to promote the NY Connects program.
  • Conduct public education sessions and outreach events to raise awareness of available services for older adults and individuals with disabilities in Brooklyn.
  • Coordinate and execute outreach strategies to increase awareness of long-term care options, support services, and public benefits available through the program.
  • Provide objective and unbiased information about long-term care options, community supports, and public services to individuals, family members, caregivers, and professionals.
  • Assist community members in understanding and applying for services and benefits, including Medicaid, SNAP, and HEAP.
  • Conduct screenings and assessments to determine eligibility for services both over the phone and in-person.
  • Facilitate referrals to appropriate services and provide follow-up to ensure access to services and client engagement has been fulfilled.
  • Use a strengths-based, person-centered approach to support individuals in achieving their care goals and promoting independence.
  • Communicate with clients in a respectful and non-judgmental manner, ensuring cultural sensitivity and understanding.
  • Advocate for clients who may encounter barriers in navigating community resources, government programs, or healthcare systems.
  • Provide ongoing care coordination for clients transitioning through various life stages or situations (e.g., hospital discharge, nursing home transitions, aging-in-place).
  • Maintain accurate case records and track outreach efforts using the NYC Aging community member database and other reporting tools.
  • Prepare periodic reports on outreach activities, service referrals, and community engagement outcomes.
  • Work closely with the Program Director and team members to strategize and enhance outreach initiatives.
  • Participate in regular team meetings, training, and professional development sessions to ensure alignment with organizational goals and NYC Aging Program Standards.
  • Perform additional tasks or responsibilities as requested by the Program Director or management team.

Qualifications and Experience

  • Bachelor's degree (BSW, BS, BA) required.
  • At least 1 year of experience in social services, human services, or a related field, preferably with older adults or individuals with disabilities.
  • Language proficiency: English (Required); Candidates fluent in Spanish, Russian, Mandarin/Cantonese, or Haitian Creole are encouraged to apply.
  • Strong written and verbal communication skills.
  • Ability to engage and build rapport with a diverse group of clients, family members, and community organizations.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
  • Knowledge of community resources, long-term care programs, and benefits such as Medicaid, SNAP, and Medicare is preferred.

Professionalism and Other Qualities

  • Ability to work independently, manage time effectively, and handle a variety of tasks simultaneously.
  • Familiarity with cultural competence and experience working with diverse populations.
  • Ability to travel independently around Brooklyn using public transportation is required.
  • This position is hybrid, with the ability to work from home 2-3 times a week, with occasional evenings and weekends required.
