Key Responsibilities:
  • Provide routine and acute care to patients, including conducting office visits, administering wellness visits, and health risk assessments.
  • Care coordination with other providers, specialists, testing facilities, and agencies to ensure seamless patient care.
  • Population health leadership, focusing on evidence-based practices and quality metrics to improve patient outcomes.
  • Educate patients on their health conditions, care plans, and treatments, promoting patient engagement and empowerment.
  • Participate in Oak Street Health promotional activities and contribute to a positive, supportive work environment.
Requirements:
  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
  • Active, non-probationary state Nurse Practitioner license and DEA license.
  • US Work Authorization and a passion for teamwork and collaboration.
Preferred Qualifications:
  • Experience in primary care, internal medicine, geriatrics, or family medicine.
  • Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
  • Bilingual proficiency in applicable areas.
