Job Summary

We are seeking a highly skilled and dedicated Nurse Practitioner to join our team at Heartland Community Health Clinic. As a key member of our healthcare team, you will provide high-quality patient care and work collaboratively with our medical staff to deliver exceptional patient outcomes.

Key Responsibilities
  • Assess patient health status and develop personalized care plans to maximize health potential.
  • Conduct diagnostic tests, prescribe non-pharmacologic therapies, and provide patient education to promote health and wellness.
  • Maintain accurate and confidential patient records, and participate in quality assurance reviews to ensure high-quality care.
  • Collaborate with healthcare professionals to provide comprehensive care and support to patients and their families.
  • Participate in ongoing education and professional development to stay current with best practices and advancements in the field.
Requirements
  • 1-2 years of experience as a Nurse Practitioner.
  • Advanced professional level of knowledge of nursing practice and theory.
  • Current State license as a Registered Nurse and licensed Nurse Practitioner, and current DEA license.
  • Ability to demonstrate knowledge and skills necessary to provide patient care appropriate to the age of the patients served.
  • Strong interpersonal skills necessary to educate patients and their families, and provide emotional support during a crisis.
