About the Role

In this position, you will work closely with discharge authorities, treatment teams, and other agencies to coordinate patient discharges and develop community-based treatment plans. Your strong communication and problem-solving skills will enable you to navigate complex situations and achieve successful outcomes.

Responsibilities

  1. Develop and implement comprehensive discharge plans that address individual patient needs
  2. Collaborate with treatment teams and other agencies to ensure continuity of care
  3. Provide education and support to patients and families during the transition process
  4. Maintain accurate records and documentation

Requirements

  • Bachelor's or Master's degree in Social Work, Psychology, Counseling, or related field
  • Eligibility for licensure as LCSW, LMFT, LPC/LMSW, LAMFT, LAPC preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
