This role is a key part of our hospital liaison program, facilitating the reintegration of clients from inpatient environments back into their communities.

About the Role

This position offers a competitive salary of $65,000 - $85,000 per year, depending on experience, with opportunities for growth and professional development. Job Responsibilities
  • Foster strong relationships with discharge authorities and other agencies to ensure seamless transitions.
  • Develop and implement effective discharge plans, taking into account individual needs and circumstances.
  • Collaborate with interdisciplinary treatment teams to coordinate community re-entry and provide ongoing support.
Requirements
  • Masters degree in social work, sociology, criminal justice, or psychology.
  • Eligibility for licensure as LCSW, LMFT, LPC/LMSW, LAMFT, LAPC is preferred.
  • Strong problem-solving and critical thinking skills, with ability to assess patient needs and develop effective solutions.
