Transition Coordinator

River Edge Behavioral Health is a leading provider of behavioral health services, seeking an experienced Transition Coordinator to join our team. This role is a key part of our hospital liaison program, facilitating the reintegration of clients from inpatient environments back into their communities.
About the RoleThis position offers a competitive salary of $65,000 - $85,000 per year, depending on experience, with opportunities for growth and professional development. Our comprehensive benefits package includes medical, dental, vision insurance, paid time off, 12 paid holidays, 401k with employer match, flexible spending accounts, short and long-term disability coverage, and life insurance.
Job Responsibilities- Foster strong relationships with discharge authorities and other agencies to ensure seamless transitions.
- Develop and implement effective discharge plans, taking into account individual needs and circumstances.
- Collaborate with interdisciplinary treatment teams to coordinate community re-entry and provide ongoing support.
- Masters degree in social work, sociology, criminal justice, or psychology.
- Eligibility for licensure as LCSW, LMFT, LPC/LMSW, LAMFT, LAPC is preferred.
- Strong problem-solving and critical thinking skills, with ability to assess patient needs and develop effective solutions.
