Job Summary

Sigma Inc is currently seeking a highly skilled Care Management Supervisor to join our team in Newark, DE. As a key member of our healthcare team, you will be responsible for providing direct oversight and supervision to our Care Management staff while ensuring exceptional patient care and outcomes.

Responsibilities:
  • Maintain accurate records and support departmental activities for the Care Management team.
  • Work collaboratively with patients, families, and healthcare teams to develop comprehensive discharge plans that meet individual needs.
  • Gather and assess information regarding patients' physical needs, mental status, family support systems, financial resources, and available community services.
  • Utilize assessment skills to develop and implement care management plans addressing patients' identified needs.
  • Ensure PTO, disability time, and FMLA oversight to optimize staffing and meet service demands.
  • Provide direct supervision, coaching, and evaluation to department staff, including orientation, mentoring, and performance reviews.
  • Evaluate staff development opportunities and competencies, performing management and evaluation tasks as needed.
Requirements:
  • Master's degree in Social Work (or related field) or BSN in Nursing (Masters preferred).
  • Minimum 5-7 years of experience in a healthcare environment, with at least 2 years serving in a leadership role.
  • Licensed Social Work credential is preferred; CCM/ACM Certification is also beneficial.
