The successful candidate will be responsible for coordinating patient care and driving progression to establish a discharge plan. This includes functioning as an interdisciplinary team member, creating, implementing, and developing all aspects of discharge planning to ensure timely and effective transitions throughout the care continuum.

Responsibilities:



  • Identify patients with post-acute care needs and develop personalized discharge plans
  • Collaborate with physicians, nurses, and other healthcare providers to ensure seamless care transitions
  • Anticipate and address potential barriers to care, participating in the development and implementation of strategies to facilitate patient progression
  • Review admission assessments and collaborate with primary nurses to ensure multidisciplinary plans of care meet identified patient needs and desired outcomes


Requirements:



  • An active RN license in DE or Compact State
  • BSN required
  • RNs with at least 2 years of medical-surgical nursing experience in an acute care hospital encouraged to apply
  • A minimum of 1 year of Care Management experience preferred
  • BLS preferred
  • Case Management Certification (CMC) required within 18 months of eligibility
