Job Summary

We are seeking a highly skilled Mental Health Co-Response Case Manager to join our team at Grand Lake Mental Health Center. As a key member of our crisis intervention team, you will work closely with local law enforcement to provide critical support to individuals in active crisis.

Key Responsibilities
  • Conduct thorough assessments of patients and their support systems to identify needs and develop effective intervention plans.
  • Collaborate with healthcare teams to ensure seamless care coordination and promote compliance with treatment plans.
  • Respond to crisis situations in the field, providing emotional support and connecting individuals with necessary resources.
  • Develop and maintain relationships with local first response agencies to ensure effective communication and coordination.
  • Participate in case reviews to evaluate case management progress and identify areas for improvement.
Requirements
  • Bachelor's degree in a related field (e.g., social work, psychology, counseling).
  • Valid Oklahoma driver's license and reliable transportation.
  • Proof of insurance and a clean driving record.
  • Ability to obtain Case Management II certification.
  • Excellent communication and interpersonal skills.
  • Ability to work non-traditional hours, including evenings and weekends.
