Grand Lake Mental Health Center is seeking a skilled Mental Health Co-Response Case Manager to join our team. As a key member of our crisis intervention team, you will work closely with local law enforcement to respond to patients in active crisis, providing assessment, planning, and interventions to help individuals cope with social, emotional, economic, and environmental problems.
Responsibilities
- Conduct thorough assessments of patients and their support systems to determine the most effective course of action.
- Develop and implement individualized treatment plans, working closely with healthcare teams and community resources.
- Collaborate with law enforcement to respond to crisis situations, providing support and guidance to individuals in need.
- Participate in case reviews to evaluate case management progress and make adjustments as necessary.
Requirements
- Bachelor's degree in a related field, such as social work or psychology.
- Valid Oklahoma driver's license and reliable transportation.
- Case Management II Certification.
- Ability to work non-traditional hours, including evenings and weekends.
