Job Summary: We are seeking a skilled and compassionate Certified Occupational Therapy Assistant to join our team at Eden Health. As a key member of our home health care team, you will provide high-quality occupational therapy services to patients in their homes throughout the Portland area.

  • Implement treatment plans ordered by physicians to patients on your caseload.
  • Review caseload with supervising OT on a regular basis to ensure best practice and state guidelines are met.
  • Evaluate assessment findings and communicate patient clinical status and ongoing needs to others in a timely manner.
  • Guarantee changes in patients' clinical status are communicated to healthcare team members, office staff, and physicians as per Agency policy.
  • Educate patients, families/caregivers, and staff on occupational therapy modalities as appropriate.
  • Deliver occupational therapy modalities tailored to patients' clinical status and needs, adhering to standard care for their age group.
  • Provide clear, concise documentation in medical records, adhering to reimbursement guidelines and Agency policy.
  • Assess and reassess pain, utilizing appropriate pain management techniques, and educate patients and families on pain management.
  • Utilize pain management modalities, including occupational therapy modalities.
  • Provide occupational therapy in an environment that prioritizes patient safety and minimizes medical/healthcare errors.
  • Participate in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, and new problems.
  • Recommend referrals to other disciplines and community resources as needed.
  • Obtain necessary supplies and equipment to provide care.
  • Support cultural practices that do not harm patients/families or interfere with planned medical treatment.
  • Teach, supervise, and counsel patients, caregivers, and family members on nursing care needs.
  • Participate in the agency's Quality Assurance Performance Improvement (QAPI) program.
  • Identify and mitigate risks to patients' safety and well-being in their environment.
  • Maintain current knowledge of community resources and access them as needed to support patient outcomes.

Requirements:

  • Meet state licensure or regulation requirements for occupational therapy assistants, if applicable.
  • Graduate from an occupational therapy assistant education program accredited by ACOTE or its successor organizations.
  • Be eligible to take or have successfully completed the entry-level certification examination for occupational therapy assistants developed and administered by NBCOT.
  • Prior home health experience is preferred.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance (MVR checked).
  • Proof of current CPR certification.
  • Able to pass a criminal background check and national sex offender clearance.
