Job Summary:

Eden Health is seeking a Certified Occupational Therapy Assistant to provide in-home services to patients in the Portland area. As a member of our team, you will have the opportunity to work one-on-one with patients, establish rewarding relationships, and make a meaningful impact on their lives.

Key Responsibilities:
  • Implement treatment plans ordered by physicians as indicated by the plan of care for all patients on caseload.
  • Review caseload with supervising OT on a regular basis to ensure best practice and state guidelines are met.
  • Evaluate the significance of assessment findings and communicate pertinent information to others in a timely fashion.
  • Guarantee that changes in patients' clinical status are communicated to other healthcare team members, necessary office staff, and physicians as per Agency policy.
  • Deliver occupational therapy modalities appropriate to the patient's clinical status and needs, and in accordance with the standard of care for patients' age.
  • Provide clear, concise documentation in medical records as it relates to reimbursement guidelines and Agency policy and procedure.
  • Assess and reassess pain, utilizing appropriate pain management techniques, and educating patients and families regarding pain management.
  • Participate in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  • Recommend referrals to other disciplines and/or community resources as needed.
  • Obtain necessary supplies and equipment to provide care, supporting cultural practices that do not harm patients or interfere with planned medical treatment.
Requirements:
  • Meets all requirements for licensure or regulation as an occupational therapy assistant by the state in which practicing.
  • Graduated from an occupational therapy assistant education program accredited by ACOTE or its successor organizations.
  • Eligible to take or successfully completed the entry-level certification examination for occupational therapy assistants developed and administered by NBCOT.
  • Prior home health experience preferred.
  • Valid driver's license and reliable transportation.
  • Proof of automobile insurance (MVR checked).
  • Proof of current CPR certification.
  • Able to pass criminal background check and national sex offender clearance.
