Occupational Therapy Assistant

Eden Health is seeking a Certified Occupational Therapy Assistant to provide in-home services to patients in the Portland area. As a member of our team, you will have the opportunity to work one-on-one with patients, establish rewarding relationships, and make a meaningful impact on their lives.
Key Responsibilities:- Implement treatment plans ordered by physicians as indicated by the plan of care for all patients on caseload.
- Review caseload with supervising OT on a regular basis to ensure best practice and state guidelines are met.
- Evaluate the significance of assessment findings and communicate pertinent information to others in a timely fashion.
- Guarantee that changes in patients' clinical status are communicated to other healthcare team members, necessary office staff, and physicians as per Agency policy.
- Deliver occupational therapy modalities appropriate to the patient's clinical status and needs, and in accordance with the standard of care for patients' age.
- Provide clear, concise documentation in medical records as it relates to reimbursement guidelines and Agency policy and procedure.
- Assess and reassess pain, utilizing appropriate pain management techniques, and educating patients and families regarding pain management.
- Participate in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
- Recommend referrals to other disciplines and/or community resources as needed.
- Obtain necessary supplies and equipment to provide care, supporting cultural practices that do not harm patients or interfere with planned medical treatment.
- Meets all requirements for licensure or regulation as an occupational therapy assistant by the state in which practicing.
- Graduated from an occupational therapy assistant education program accredited by ACOTE or its successor organizations.
- Eligible to take or successfully completed the entry-level certification examination for occupational therapy assistants developed and administered by NBCOT.
- Prior home health experience preferred.
- Valid driver's license and reliable transportation.

- Able to pass criminal background check and national sex offender clearance.
