Job Summary:The Dietitian - Level II is responsible for providing direct comprehensive Nutritional Education and Counseling to Pediatric and Adolescent patients. The successful candidate will work closely with the Associate Director of Chronic Disease Management to develop and implement care plans for high-risk individuals. The Dietitian - Level II will also provide nutritional educational handouts based on identified areas of concern and document patient encounters in the Electronic Medical Record (EMR).
Responsibilities:- Provide nutrition education, assessment, documentation, and intervention to patients.
- Develop care plans for high-risk individuals.
- Provide nutritional educational handouts based on identified areas of concern.
- Document patient encounters in the EMR.
- Participate in team meetings on chronic diseases.
- Participate as a Health Coach for Diabetes Prevention Program.
- Baccalaureate degree from an accredited college or university, with a major in Dietetics or Foods and Nutrition in a program approved by the American Dietetic Association (ADA); or registration or eligibility for registration by the Commission on Dietetic Registration (CDR) as a Registered Dietitian or New York State Certification as a Dietitian-Nutritionist.
- 1-2 years' experience or more working in an Ambulatory Care Setting with pediatric and adolescent patients.
