At General Physician, P.C., we are committed to providing high-quality patient-centered care to improve the overall quality of life for our patients. We are seeking a skilled Registered Nurse to join our Primary Care office in Hamburg, NY.

Key Responsibilities:
  • Assist providers with the administration of patient care, ensuring a safe and healthy work environment.
  • Communicate effectively with patients, providers, and other healthcare professionals to ensure seamless care coordination.
  • Perform telephone triage, assessing patient needs and communicating health information to providers.
  • Follow service line initiatives to prevent unnecessary hospital re-admissions, collaborating with appropriate parties to ensure proper follow-up.
  • Assist with medication management, performing medication reconciliation and queueing necessary refills for providers.
Requirements:
  • Applicable NY State license, with a two-year degree preferred.
  • Proven work experience in a healthcare facility, with basic knowledge of ICD-10 and CPT Codes, CPR Certification, and strong clinical skills.
  • Strong written and verbal communication skills, with the ability to utilize computer programs.
