Integrative Health Medicine Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Integrative Health Medicine team at Hackensack Meridian Health. As a key member of our team, you will provide exceptional patient care, utilizing a patient-centered coordinated care model to address the whole person's health and well-being.
Key Responsibilities- Assess, plan, and implement care for individuals with health and safety needs, working collaboratively with other practice/hospital team members.
- Utilize integrative health medicine experience to address the root cause of symptoms, evaluating the 5 pillars of Health and Well-Being: Sleep, Nutrition, Activity, Resilience, and Purpose.
- Listen deeply to patients' histories, identifying interactions among genetic, environmental, and lifestyle factors that can influence long-term health.
- Design and manage programs that optimize well-being and support patient-centered, holistic non-pharmacological approaches to symptom and disease management.
- Perform clinical care professionally, serving as a customer advocate and role model, adhering to NJ Board of Medical Examiners regulations, the Code of Ethics, NJ Department of Health requirements, and HMH policies and procedures.
- Develop and implement the appropriate plan of care, facilitating patient teaching and support programs for patients and employers.
- Evaluate current protocols and standards of care, supporting the development of evidence-based practice protocols and standards of care.
- Apply evidence and value-based practices to achieve positive outcomes for patients, collaborating with physicians and consulting effectively with other members of the healthcare team.
- Provide accurate, systematic, and organized documentation of findings, recommendations, plans, and education.
- Participate in continuing education and maintain documentation, developing a personal clinical expertise development plan.
- Serve as a clinical resource to nursing staff, patients, and other multidisciplinary team members, promoting excellence in care and clinical outcomes.
- Promote a collaborative work environment that supports the organizational mission, behavior expectations, and values.
- Graduate of an NLN/AACN accredited program in nursing.
- Master's Degree.
- Integrative Health and Medicine and/or functional medicine training.
- HeartMath trainer or willing to work towards certification.
- Experience in relevant patient care settings or as regulations dictate.
- Master's Prepared Advanced Practice Nurse.
- Minimum of 3 years professional experience.
- Membership in pertinent holistic nursing and integrative health professional organizations.
- Strong organizational and computer skills.
- Effective verbal and written communication skills.
- Emotional intelligence, critical thinking, and effective interpersonal skills.
- Ability to adapt to multiple and changing priorities.
- Valid Driver's license.
- Current valid, unencumbered NJ State Professional Registered Nurse license.
- Maintains certification as an APN in New Jersey.
- Maintains American Heart Association Basic Life Support HCP certification.
