Key Responsibilities- Deliver Patient-Centered Care: Provide evidence-based nursing practices to ensure safe and excellent delivery of individualized patient care.
- Collaborate with Healthcare Team: Communicate effectively with patients, nursing colleagues, and other healthcare professionals to ensure seamless care coordination.
- Medication Administration: Utilize advanced technologies to ensure the safe and accurate administration of medications.
- Electronic Documentation: Maintain accurate and up-to-date electronic records of patient assessments, care plans, and interventions.
- Patient Education: Provide patient-centered education to optimize health, wellness, and disease management.
- Discharge Planning: Ensure safe transitioning and continuum of care for patients through multi-disciplinary discharge planning.
- Current NJ Nursing License: Possess a valid and current New Jersey nursing license.
- BLS/CPR Certification: Hold BLS/CPR certification from the American Heart Association within 60 days of hire.
- Nursing Experience: Have a minimum of 6 months of nursing experience or be an active participant in the Hackensack Meridian Health Nurse Residency program.
- Telemetry Certification: Possess telemetry certification (preferred).
- ACLS, NIH Certifications: Hold ACLS and NIH certifications (preferred).
- Step Down Progressive Care Experience: Have experience in step down progressive care (preferred).
- BSN Degree: Possess a BSN degree (preferred).
- Strong Communication and Teamwork Skills: Demonstrate exceptional communication and teamwork skills, with a strong sense of accountability for improving patient outcomes.
