Job Title: Licensed Practical Nurse

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at ChenMed. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our organization.

Job Summary:

The LPN will work under the supervision of a physician to provide general nursing care to patients, assist with exams and procedures, and maintain accurate medical records. The ideal candidate will possess excellent clinical skills, strong communication abilities, and a passion for delivering exceptional patient care.

Key Responsibilities:
  • Provide general nursing care to patients, including vital sign checks, medication administration, and wound care.
  • Assist physicians with exams and procedures, including phlebotomy and lab specimen collection.
  • Maintain accurate and up-to-date medical records, including patient histories and test results.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Current, active LPN license to practice in the state of employment.
  • Graduation from a nationally accredited school for practical or vocational nursing.
  • Minimum of 1 year of work experience as an LPN, with a preference for experience working with geriatric patients.
  • EMR system experience is preferred.
